Open the correct visit, then Workspace → Ultrasound. The ultrasound's standard fields are part of the Visit Record. They are organized into Info, Sonographer Notes, Physician Review, and Baby 1–3.
Upload ultrasound images
In the same visit, choose Files.
Select Upload files.
Choose Ultrasound in File under.
Select the images or scan files and wait for each upload to finish.
Open the files to confirm that the correct images are attached.
The upload dialog supports images, PDFs, DICOM scans, and other visit documents. Each file you select uploads on its own, so a full scan session isn't limited to a small batch. Every file still has to be under 100 MB. If an upload fails, use the error shown for that file before trying to send the review.
If one image shows a broken file icon instead of the picture, especially when you check it on a phone or tablet, that usually means that one file didn't finish uploading or was damaged, not a problem with your device or the other images on the visit. Delete that file from Files and upload it again. If the same file still shows a broken icon after a fresh upload, contact CoolFocus support with the file name so they can check the file and your account.
New uploads are private. Use Share with client only when you intend that file to be available in the Client Portal. Uploading for a physician's review and sharing with a client are separate actions.
Record the findings
Use Info for the exam details and observations. Use Sonographer Notes for the sonographer's narrative. The Physician Review area contains the physician's review information and may be restricted by permissions.
In Info, the Sonographer field lists the same staff as Physician and Ultrasound Consultant: the people your organization has set up as consultants. Names are listed alphabetically by last name. If a staff member you expect to see is missing from any of these three lists, confirm their account is set up as a consultant before assuming they were removed.
Baby 1, Baby 2, Baby 3
Each Baby section repeats the same set of biometry fields, so staff can chart multiple fetuses separately on the same visit:
Gestational sac (GS) and its gestational age
Crown-rump length (CRL) and its gestational age
Biparietal diameter (BPD) and its gestational age
Head circumference (HC) and its gestational age
Occipito-frontal diameter (OFD) and its gestational age
Abdominal circumference (AC) and its gestational age
Humerus length (HL) and its gestational age
Femur length (FL) and its gestational age
Estimated fetal weight (EFW) and its gestational age
Fetal heartbeat observed?
Fetal heart rate (BPM)
For a single-baby pregnancy, chart the fetus under Baby 1 and leave Baby 2 and Baby 3 blank.
Send the Doc Alert
Doc Alert is the built-in physician review of the ultrasound. To send it:
Finish the findings and confirm the uploaded images.
In Ultrasound → Info, select Physician.
Select Notify User After Doctor Signs. This is the staff member who should hear when signing is finished; it is not a second physician selection.
Save the visit.
Choose Send to Physician and read the result.
A physician and notification staff member must both be selected. If either selection, or another required step like marking the ultrasound as performed, is still missing, Send to Physician stays visible but disabled, and shows the exact reason next to it so you know what to fix before sending. If the response warns that the reviewer cannot be notified, confirm their account and email with an administrator and contact the reviewer directly.
Check what happened
The Doc Alert group in Info shows Sent to Physician, Sent to Physician Date, Reviewed by Physician, and Physician Signed Date. These are status fields, not checkboxes staff use to approve their own work.
Use Review status for more detail. In Approvals, Submitted by me tracks work you sent; Awaiting Approvals is work assigned to you as a reviewer. A sender can have an empty Awaiting Approvals list while the physician still has work to do.
Corrections and reset
Read the reviewer's reason when a review is returned, then use the correction/resubmission actions available on that record.
Reset Doc Alert is a separate recovery action. After a Doc Alert has been sent or reviewed, an administrator must perform the reset. Resetting a signed Doc Alert clears its current sign-off and requires another review. It is not a way to mark a review complete.
If you cannot find the action mentioned in a help article, confirm whether you opened the Visit Record's Ultrasound section or a chart document. Those surfaces can offer different correction controls.
Print or save the ultrasound record as PDF
Print/PDF is an action on the ultrasound's own chart entry. It does not sit on the inline Info, Sonographer Notes, or Baby fields you edit on the Visit Record.
Open the visit and go to Workspace → Ultrasound.
Open the ultrasound chart entry itself, rather than staying on the inline Visit Record fields.
Choose Print/PDF from that entry's action bar.
Viewing or printing this PDF needs View access to the Ultrasound chart type, set on Settings → Security → User Groups. See Managing Chart Templates for what the printed record includes.
If your user group already has View access to Ultrasound and Print/PDF still doesn't appear on a record, contact support. Support has found a small number of ultrasound records without a standalone printable entry, and is tracking it as a bug.
