Custom Assessments turn answers into points, then map each score to a named Result, such as Low, Moderate, or High. If your team calls these results levels, result ranges are where you set them up.
Availability: Custom Assessments are included with CPC Premium. You also need organization administrator or module administrator access to manage templates.
Open the assessment template
Go to Settings → Care → Intake & Documents → Assessments.
Open an existing template, or select Create to make a new one.
On the Questions tab, make sure each scored answer has the intended point value. If you want separate results for different parts of the assessment, assign questions to domains.
Open Result Calculation.
Assessment templates save automatically. There is no separate Save button.
Choose how CoolFocus calculates the score
Select the calculation that matches how your assessment should work:
Add up the points — adds the points contributed by all scored answers.
Weighted total — applies each question's weight when calculating the total.
Average of answers — calculates the average point value of the answered questions.
Highest single answer — uses the highest point value from any answered question.
You can create a result for each domain and, if useful, an overall result across the whole assessment. Turn off Overall result when only the domain results are meaningful. Use Show the running result to staff while they fill this out if staff should see the result update as they answer questions.
Add result ranges
Select Add range.
Enter the inclusive From and To scores.
Give the result a clear name, such as Low, Moderate, or High.
Choose a color to help staff recognize the result.
Add a recommendation that tells staff what to do next.
Repeat until the ranges cover every possible score.
For an assessment with a possible total of 0–12, the ranges might look like this:

Moderate | 4 | 7 | Review the responses and discuss next steps. |
High | 8 | 12 | Follow your organization's high-priority response plan. |
Range endpoints are inclusive. In this example, a score of 3 is Low and a score of 4 is Moderate.
Avoid gaps and overlaps. A score in a gap has no matching result. If ranges overlap, CoolFocus uses the first matching range. The builder warns about these problems, but the warning does not prevent autosave.
Preview and verify the results
Open the Preview tab.
Choose answers that produce the lowest and highest possible scores.
Test each boundary where one result changes to the next. For the example above, test scores of 3 and 4, then 7 and 8.
Confirm the result name and color. Review each recommendation in Result Calculation; recommendations appear when staff view a completed assessment.
If you later change answer points, reverse scoring, question weights, domains, or the calculation method, revisit the ranges. Those changes can alter the possible score.
Troubleshoot a result
A score has no result: Check for a gap or a final range that stops below the highest possible score.
The wrong result appears: Check for overlapping ranges, then review the answer points, reverse-score settings, weights, and calculation method.
The Assessments setting is missing: Confirm that your organization has CPC Premium and that you have organization administrator or module administrator access.
Changes do not rewrite assessment history. Template edits apply to assessments saved after the change. Existing assessments keep the questions, scoring rules, and result labels that were in place when they were first saved, whether as a draft or completed.
