Since you can create your own Sources ("How Did you Hear About Us") in CoolFocus, we list ALL of your Sources in our Care Net report that were assigned to clients during the time period you run the report for. That way, you can pick the top three from your list that correspond to the Care Net list.

This number pulls all entries of 6 and above entered on the Exit Survey tab of the Visit. Note: This data cannot be entered in CoolFocus for a new Client or a new Case until the Intake Form has been submitted.

Care Net Definition: A “client” is any person who physically visited the pregnancy center, completed an INTAKE and received a service such as a pregnancy test, options counseling, STD testing, etc. For the purposes of this report, individuals who ONLY interacted with the center via email, instant messaging, texting, telephone, etc. should NOT be reported here.

CoolFocus Report to verify: New Cases report

Requirements: A Case Initiated Date within the Date Range specified, Client marked as “MALE” or “FEMALE” in Demographics, the FIRST VISIT for the Case is also within the Date Range specified, and that Visit occurred at the Center Location specified.

New Clients, completed ONSITE Intakes - includes Clients who had Appointment or Walk-In selected in the "Today's Visit was" field on the Client Provided Info tab of the Intake Form.

New Clients, completed VIRTUAL Intakes - includes Clients who had Virtual selected in the "Today's Visit was" field on the Client Provided Info tab of the Intake Form.

This data is calculated based on the selection in the "Client is Father Of Baby" field on a Male Client's record. 

View an existing Male Client's record and click "View Active Case" in the Actions box on the right.  Then click the Edit tab. You will see the "Client is Father Of Baby" field on the Case Info tab.

CoolFocus Report to Verify: Clients by Marital Detail (NOTE: This report includes ALL CLIENTS who had a VISIT in the Date Range specified, and not just Clients who had a CASE INITIATED date within that date range. This means you may have MORE Clients on this report than are listed on the Care Net Marital Status count.)

Requirements: A Case Initiated Date within the Date Range specified, a Visit for the Case is also within the Date Range specified, and that Visit occurred at the Center Location specified.

Single Clients - this count includes Single, Widowed, Divorced, Engaged, and Cohabitating.

Married Clients - this count includes Married and Separated.

Unknown Marital Status - this count includes all other Clients.

CoolFocus Report to Verify: None. The Care Net report calculates Age of Clients based on the Start Date selected for the Care Net report, and not based on her actual birthdate. This is to remove Clients from having two different Ages if they have two different Cases during the year when she may have had a birthday.

The Clients by Age report in CoolFocus, however, calculates the Age of the Client based on the Case Initiated Date, which will explain any difference you may see in the age statistic on this report vs. the Care Net report.

Requirements: A Case Initiated Date within the Date Range specified, a Visit for the Case is also within the Date Range specified, and that Visit occurred at the Center Location specified.

CoolFocus Report to Verify: Clients by Race Detail. (NOTE: This report includes ALL CLIENTS who had a VISIT in the Date Range specified, and not just Clients who had a CASE INITIATED date within that date range. This means you may have MORE Clients on this report than are listed on the Care Net Ethnicity count.)

"Native Hawaiian/Other Pacific Islander" counts all New Clients who have "Asian/Pacific Islander" selected in the Race field on the Client Provided Info tab of the Intake Form (Demographics tab of the Client record).

Asian has been added to list of selections in CoolFocus.

CoolFocus Report to Verify: Visit Details Report. (What Care Net calls a "Session," CoolFocus calls a "Visit.") Export the Visit Details report into Excel and sort Column Y to show Virtual visits verses the other visit types.

Requirements: Any Visit within the Date Range specified.

CoolFocus Report to Verify: Pregnancy Test Results Details. Use the “Offsite?” column to remove any Tests that have “True” in this column. You can also use the “Excel Data Only” version of this report to remove those tests.

Requirements: Pregnancy Test is associated with a Visit within the Date Range specified, and does NOT have the “Test Performed Offsite” checkbox checked.

NOTE: ALL Onsite Tests are included, and not just the ones for Cases Initiated within the Date Range provided. ONLY Tests with “Positive,” “Negative,” “Inconclusive,” or “Results Pending” are included. Tests MUST be associated with a Visit that occurred at the Center Location selected.

CoolFocus Report to Verify: Ultrasound Results Details

Requirements: Total number of Ultrasounds during the Date Range provided. The Ultrasound MUST be associated with a Visit that occurred at the Center Location selected. ALL Ultrasounds are included, and not just for Cases with an Initiated Date within the Date Range provided.

CoolFocus Report to Verify: Ultrasound Results Details (remove any RE-SCANS from this count).

Requirements: An Ultrasound provided that was associated with a Visit during the Date Range provided. This counts the DISTINCT CLIENTS and removes re-scans; it is only counting PEOPLE, not the total number of individual ultrasounds. The Ultrasound MUST be associated with a Visit that occurred at the Center Location selected. ALL Ultrasounds are included, and not just for Cases with an Initiated Date within the Date Range provided. ALSO: ONLY Cases that have a POSITIVE PREGNANCY TEST recorded ONSITE will be included. If the Case does NOT have a Positive Pregnancy Test OR the Test is marked as "Test Performed Offsite," then that Client will NOT be counted.

CoolFocus Report to Verify: Ultrasound Results Details (remove any RE-SCANS from this count), and run this report for all Mobile Ultrasound Center locations you have.

Requirements: An Ultrasound provided that was associated with a Visit during the Date Range provided. This counts the DISTINCT CLIENTS and removes re-scans; it is only counting PEOPLE, not the total number of individual ultrasounds. The Ultrasound MUST BE associated with a Visit that occurred at a Center Location that has the Center Type of "Mobile Ultrasound." ALL Ultrasounds are included, and not just for Cases with an Initiated Date within the Date Range provided. ALSO: ONLY Cases that have a POSITIVE PREGNANCY TEST recorded ONSITE will be included. If the Case does NOT have a Positive Pregnancy Test OR the Test is marked as "Test Performed Offsite," then that Client will NOT be counted.

NOTE: You should only report this number with your Main Center report, and NOT with other "brick and mortar" center locations. Care Net considers a Mobile Ultrasound Unit as part of the Main Center, and NOT its own "location." When you run your Care Net Renewal report for your MAIN CENTER, be sure to ALSO select any Mobile Ultrasound center locations you may have.

Requirements: An Ultrasound Visit that has the "Father Present at Ultrasound?" box checked.

Requirements: This counts the DISTINCT CLIENTS (not the VISITS) who had a Primary OR Additional Visit Reason of “Pregnancy Options/Parenting Counseling” for a Visit that occurred within the Date Range provided, OR that had a Program associated with the Visit that has the Care Net Associated Program of “Pregnancy Options Counseling.” The Visit must have occurred at the Center Location selected.

Requirements: This counts the DISTINCT CLIENTS (not the VISITS) who had a Primary OR Additional Visit Reason of “Post-Abortion help” for a Visit that occurred within the Date Range provided, OR who had a Program associated with the Visit that has the Care Net Associated Program of “Post-Abortion Support.” The Visit must have occurred at the Center Location selected.

Requirements: This counts the DISTINCT CLIENTS (not the VISITS) who had a Primary OR Additional Visit Reason of “Abstinence Counseling” for a Visit that occurred within the Date Range provided, OR who had a Program associated with the Visit that has the Care Net Associated Program of “Abstinence Counseling." The Visit must have occurred at the Center Location selected.

This counts the DISTINCT CLIENTS (not the VISITS) who had an STI Test(s) provided.

CoolFocus Report to Verify: Distinct Clients - click the Export button and total column BG in the Excel spreadsheet created of all test dates in the renewal year.

To start tracking this data for 2025, create a Role (in Validation Tables) of Chemical Abortion. Assign this Role to Clients as needed.

Requirements: This counts the DISTINCT CLIENTS (not the VISITS) who had a Primary OR Additional Visit Reason of “Baby/Maternity Supplies” or “Material Service Only” for a Visit that occurred within the Date Range provided, OR who had a Program associated with the Visit that has the Care Net Associated Program of “Clothing or Material Resources.” The Visit must have occurred at the Center Location selected.

Requirements: This count depends on whether your Center uses Resources Provided or Program Participation to log Material Resources. Please use either the "Resources Provided" report or the "Program Participation Program Summary" report.

Requirements: This counts the DISTINCT CLIENTS (not the VISITS) who had a Primary OR Additional Visit Reason of “Parenting Class” for a Visit that occurred within the Date Range provided, OR who had a Program associated with the Visit that has the Care Net Associated Program of “Parenting Education.” The Visit must have occurred at the Center Location selected.

Requirements: This counts the DISTINCT CLIENTS (not the VISITS) who had at least ONE Visit that had “Was the Gospel Presented” checked. The Visit must have occurred at the Center Location selected.

Requirements: This counts the DISTINCT CLIENTS (not the VISITS) who had at least ONE Visit that had a Post-Visit Spiritual Assessment of “Made a New Profession of Faith” or "Rededication." The Visit must have occurred at the Center Location selected.

Requirements: This counts the number of churches that have a Role of "Care Net: Active Church."

Requirements: This counts the number of churches that have a Role of "Care Net: Active Church" AND "Care Net: Referral Church."

Requirements: This is the total number of Clients from 56B and 56C.

Requirements: This counts the number of clients who have a Visit with a Resource Provided of "Care Net: Church Referral Permission-Based."

Requirements: This counts the number of clients who have a Visit with a Resource Provided of "Care Net: Church Referral Making Life Disciples."

C. Of total in B, this count includes those Clients who had the "Father Present at Ultrasound?" box checked on the Ultrasound tab of the Visit.

Last stated intent: Abort - Total Distinct Clients who had a KNOWN OUTCOME of “Abortion” OR a Last Stated Intention of “Abortion.”

Last stated intent: Carry to term - Total Distinct Clients who had a KNOWN OUTCOME of “Carry,” “Parent,” “Adoption,” etc. OR a Last Stated Intention of “Parent,” “Carry,” or “Adoption.”

Last stated intent: Undecided or Unknown - Total Distinct Clients who had an Unknown Outcome OR Last Stated Intention of “Undecided” or “Unknown.”

This information comes from the "Case Info" tab on the Intake Form, or from the Case Edit page. Make a selection in the "Fathers Relationship" field in the Relationship with Potential Father section.

This question counts the number of unique Donors who gave at least one time during the previous year. The answer to A, B and C is based on the ENTITY TYPE assigned to your Donors. Since this is NOT a required field, you may have Donors who do NOT have an Entity Type assigned.
